Palliative Care Defined:
Palliative care is specialized medical care conducted by nurse practitioners in the comfort of your home (covered by insurance). These in-home visits aim to focus on symptom relief and improving quality of life for the patient and family by: Reducing unnecessary hospital visits, prescribing medications, providing pain and symptom control, acute care, continuing patient education, advance care planning and medication management.
We put the patient's goals and desires first in hopes of making life more comfortable to live, increasing quality time with their families, and overall reducing patient and family burden. There is no referral needed to start our services!
Who is Palliative Care for?
Palliative care is specialized medical care for people living with a serious illness. It's provided by a specially-trained team of nurse practitioners, social workers and other specialists who work in partnership with a patient's existing doctor and care team to provide an extra layer of support.
Palliative services are based on the needs of the patient, not necessarily on the patient’s prognosis. It is appropriate at any age and at any stage of a serious illness. As an added benefit, palliative care can also be provided concurrently with ongoing curative treatment.
How is it different from hospice?
Palliative Care:
•Available at any stage of the disease process
•Occurs at the same time as existing health services
•Can co-exist with home health and therapy such as:
- physical therapy
- ongoing curative therapy
- specialized treatment
Hospice:
•Only available when life expectancy is six months or less
•Excludes curative/aggressive treatments
•Hospice cannot coexist with home health or therapy such as physical/occupational therapy services
