At Home Palliative Care's goal is to provide services and comfort to those suffering from a life-limiting illness while improving quality of life. This is accomplished by managing the symptoms to decrease the patient's burden and discomfort.
We specialize in symptom management and palliative services but are not directly tied to hospice. This means our team will help with a hospice transition when it's appropriate but will not pressure you into a situation that may not fit your immediate needs.

Advance Care Planning is not just for elderly or those under palliative services. At any age, a medical crisis could leave you in a situation where you are unable to make your own health care decisions. Even if you are not in poor health, planning for deliberate care in the future is an important step toward making sure you get the treatment you want...on your own terms. If you are left unconscious or unable to speak for yourself, doctors and family members are navigating decisions about a treatment plan for you without knowing your intent or wishes.
